Norton Sound Health Corporation Primary Care Coordinator Nome, Alaska Purpose of Position: Perform skilled administrative duties in support of the Patient Centered Medical Home initiative. Work closely with the Primary Care Manager and the Wellness Director providing support for day-to-day activities. Tasks may include, but are not limited to, inputting sensitive information into information systems, preparing correspondence, researching and formatting information, and performing analysis and recommendations based on information retrieved. Uphold the organization's vision, mission, and corporate values. Demonstrate understanding of and compliance with the organization's policies, procedures, code of conduct, and work rules. Essential Functions: Works in conjunction with the Med Staff Coordinator to maintain and update the Primary Care Medical Staff Master Schedule, the Primary Care Provider Shift Board Schedule, and other scheduling platforms in coordination with the Primary Care Clinic Manager, Medical Staff Physician Administrator, and Medical Director. Provides Scheduling and Case Management Support for the Primary Care Clinic during times of short staffing. Collect and distribute Primary Care Clinic mail and confidential information between departments. Coordinate with Primary Care Staff for timecards, travel, etc. Coordinate meetings between multiple departments as needed in support of the Patient Centered Medical Home. Record and edit the minutes of meetings and distribute to appropriate officials or staff members. Issue staff notification of all official activities or meetings. Record and maintain all vital meeting records. Prepare meeting agendas or packets of related information. Prepares all necessary meeting items so that they can be executed, recorded, archived, or distributed. Assist with hiring and onboarding process for new staff. Provides department-specific orientation in collaboration with the Primary Care Manager to help orient new staff to Primary Care, other departments, and facility introductions. Collect, organize, and get processing approval for Check Requests, Continuing Education Training Recaps, and Travel Recaps for all Primary Care Staff. Assist with purchase orders and tracking of purchased items. Assist with typing of Policy and Procedures, meeting minutes, and other documents as designated. Assist with maintaining employee files to include but not limited to tracking competencies and ensuring necessary documents are included in the employee's HR file. Assist Clinic Manager and Charge Nurse with maintaining staffing schedules, communicating with staff as needed regarding changes and staffing needs and provider clinic schedule changes that affect staffing. Assists PCC and Med staff with panel management for Teams. Performs other duties as assigned.
