The University of Miami/UHealth Department of Otolaryngology is currently seeking a full time Sr. Patient Navigator to work in Miami, FL. The Senior Patient Navigator provides support and guidance throughout the patient’s care continuum by helping patients “navigate” through the maze of doctors’ offices, clinics, hospitals, outpatient centers, insurance and payment systems, patient-support organizations, and other components of the healthcare system. The Senior Patient Navigator also acts as a resource for lower-level Patient Navigators to ensure the timely delivery of quality care and patient satisfaction.

CORE JOB FUNCTIONS

  • Maintains a working knowledge of medical symptoms, signs, and anatomical systems to identify and differentiate type and urgency of medical need.

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.

  • Ensures patients have access to insurance and social service programs for which they are eligible.

  • Maintains knowledge of insurance referral requirements to ensure access based on third party reimbursement criteria.

  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.

  • Provides patients with all required information regarding appointments and payment policies (i.e., parking, cash policies, anticipated charges, required ancillary services, cancellation policy).

  • Identifies and resolves real or perceived barriers to care.

  • Intervenes as liaison/advocate for patients, physicians, and staff in facilitating ease of care. Assists in identifying trouble spots and problem patterns in the provision of care.

  • Educates and counsels patients on issues related to their health and life situations.

  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.

  • Assists in building partnerships with local community physicians and referral sources to the UHealth systems.

  • Assists lower level Patient Navigators as needed.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

CORE QUALIFICATIONS

Education:

High School diploma or equivalent

Experience:

Minimum 4 years of relevant work experience

DEPARTMENT ADDENDUM

Department Specific Functions

  • Serve as support to patients who are initiating their care with the Children’s Hearing Program (recently diagnosed children with hearing loss); support is defined as ensuring they have the information necessary to adhere to an optimal care plan, as created by the provider(s).

  • Working on a check-in for patients newly diagnosed with hearing loss by calling families 3-5 days after their appointment to ensure they understand next steps.

  • Guiding families and advising them of ancillary family support services (social work, psychology, education) should this be needed or requested.

  • Work closely with administrative teams to ensure patients are ready for their visit; this includes sharing any relevant information with patients and ensuring authorization is sought primarily through communication with the CIV teams.

  • Serve as a liaison between the Children’s Hearing Program and the Holtz newborn nursery to coordinate outpatient hearing screenings following a failed newborn hearing screening.

  • Navigate patients through account and insurance registration with UHealth system

  • Ensure all patients have appropriate instructions prior to sleep-deprived test

  • Support families as they navigate the cochlear implant process. This process is lengthy which includes several appointments, the Patient Navigator will ensure that all needed appointments have been scheduled and authorized.

  • Patient Navigator will call families after qualifying diagnosis for cochlear implants

  • Patient Navigator will advise families of all required appointments within the Children’s Hearing Program

  • Patient Navigator will support families navigating the need to obtain imaging at neighboring pediatric facilities (NCH, Memorial, etc – as UM does not provide imaging for babies)

  • Serve as the liaison between patient and providers as needed.

  • Help patients and their families in obtaining referrals to a specific specialists (genetics, ophthalmology, neuro, etc).

  • Patient Navigator will accomplish other relevant duties and responsibilities as assigned.
