As the Community Director- LPN at Fox Trail Memory Care Living, you will have the opportunity to make a significant impact on the lives of our residents and their families. We are seeking a compassionate and dedicated individual to lead our team and ensure the highest quality of care for those in our community.
**Job Overview:**
As the Community Director- LPN, you will be responsible for overseeing the daily operations of our memory care community. You will work closely with residents, families, and staff to create a warm and inviting environment that promotes independence and dignity for all.
**Responsibilities:**
- Provide clinical oversight and support to our care team
- Develop and implement care plans for residents
- Coordinate and communicate with families and healthcare providers
- Ensure compliance with state regulations and company policies
- Lead and mentor staff to deliver exceptional care
- Manage budget and resources effectively
**Requirements:**
- Active LPN license in the state of
- Minimum of 2 years of experience in a leadership role in a healthcare setting
- Strong communication and organizational skills
- Ability to work collaboratively with a diverse team
- Understanding of memory care best practices
- Compassionate and patient-centered approach to care
