Job Summary
This position is responsible in a clinical advisory role to assess members for wellness education and disease management; introduce members to utilize the personal health manager website as a tool for members to better manage their health; educate members regarding wellness and specific condition; facilitate coordination of care for members identified through claims; completion of the HRA; and self-referral/ referral from the um and/or cm departments, or physician referral. If a registered nurse (RN), may respond to web based ask a nurse questions.
This position is Remote / Telecommute
JOB REQUIREMENTS:
* Current, unrestricted license in the state of operations in one of the following: Registered Nurse (RN), or Licensed Professional Counselor (LPC), Registered Dietician (RD) or Licensed Master Social Worker (LMSW)..
* 3 years clinical experience.
* Patient education experience.
* Computer/system software experience including Web.
* Verbal and written communication skills; analytical skills; sound clinical judgment.
* Incumbents with nursing licenses in positions/departments requiring multi-state licenses are required to obtain and maintain additional current, valid, and unrestricted applicable nursing licenses in other states as determined by management. Multi-state license fees will be provided by HCSC. Incumbents with other clinical licenses are not required to obtain multi-state licenses.
Preferred Job Requirements:
* Registered Nurse preferred
* Disease Management experience to include Diabetes, Asthma, Congestive heart failure, Coronary artery disease and COPD.
* Experience with Autoimmune disorders, neurological diseases, Gastroenterological diseases & blood disorders preferred.
* CCM preferred or able to obtain within one year.
