The service coordinator may assess service needs and determine eligibility for public services.What You Will Do: This job description should not be interpreted as all inclusive. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential job functions.Provide general case management (including intake) and referral services to all residents needing such assistance.This may include formal case management (i.e. evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation) for a tenant when such service is not available through the general community.This may also include developing case plans in coordination with assessment services in the community or with a PAC.Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate.Establish connections with all agencies and service providers in the community; determine most fiscally responsible in services to assure individualized, flexible, and creative services for the involved tenant(s).Create and maintain a directory of providers for use by tenants and project staff. Refer and/or connect tenants to programs and services at The Salvation Army Eagle Creek Corps, as well as other service providers in the general community, i.e. case management, personal assistance, homemaker, Meals-On-Wheels, transportation, counseling, spiritual and emotional care, intermittent medical care, preventative health care, and legal advocacy, etc.Educate tenants regarding service availability, application procedures, client rights, etc., providing advocacy as appropriate.Assist tenants in building informal support networks with other residents, family, and friends.Additional responsibilities as needed:Set up volunteer programs with service organizations in the community.Provide or coordinate training in the obligations of tenancy.Educate staff on the management team on issues related to aging and service coordination to help better assist tenants.Project Responsibilities:Perform office filing.Assist in budget preparation process.Represent the project at local and national conferences.Assist property management company with daily operations as needed.Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position.Education: Bachelors degree in Social Work, Gerontology, Psychology, or Counseling preferred. Background Checks: Position requires a background check to be completed, and all background check results will be reviewed.Experience: 2-3 years in social service delivery.Certifications: Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe From Harm training within the first 90 days of employment. Must complete Caseworker Certification Program within 120 days of employment. Must have Blood Born Pathogens Classification "A". Skills/Abilities: Clear understanding of The Salvation Army and willingness to support its mission.Training in the aging process, elder services, disability services, eligibility for and procedures of federal and applicable state entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol abuse by the elderly, and mental health issues is a plus.Demonstrated working knowledge of supportive services and other resources for senior citizens and the non-elderly disabled in the area served by the project.Demonstrated ability to advocate, organize, problem-solve, and provide results for the elderly and the disabled.Able to speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele.Computer proficiency with Microsoft products and ability to learn electronic reporting systems.Supervisory Responsibility: No supervisory responsibilities.The physical requirements described here are representative of those that must be met by an employee to successfully perform the duties of this job.
