POSITION SUMMARY

We are pleased to offer the job opportunity of the Sub Acute Clinical Operations Manager in the DuPage County Care Center. This position is responsible for managing the daily operations of the Sub Acute unit to ensure quality clinical care, patient satisfaction, and staff compliance with organizational policies and procedure. This position serves as a member of the DuPage County Care Center (DPCC) nursing management team.

RESPONSIBILITIES

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supervises Nurse Liaison and provides facility clinical approval criteria.
  • Actively participates in Northwestern Medicine Post Acute Transitions meetings and evaluates unplanned rehospitalizations and other clinical protocols.
  • Coordinates resident care, manages, and supervises residents and staff in Covid unit/PUI unit in addition to Post Acute unit.
  • Evaluates and coordinates training unit staff on new medical technologies.
  • Performs as a liaison between management and clinical care including providing updates to Nursing Administration regarding sub-acute operations
  • Coordinates daily operations and oversees patient care operations, assigning and monitoring nursing staff, and identifying and implementing quality improvement on the unit to ensure quality clinical care and patient satisfaction
  • Plans, outlines, evaluates, and supervises the work of professional and sub-professional personnel on the unit, personally advising and assisting with the more difficult situations.
  • Participates in the process of interviewing and hiring new personnel, and disciplines or effectively recommends such action for assigned staff including the MDS Coordinators and Administrative Staff
  • Recommends hires and promotions, directs, and evaluates employment decisions for all assigned positions
  • Support nursing staff in other units with critical thinking skills to anticipate, assess and intervene in an acute care environment
  • Maintains quality care by reviewing the daily census, staffing, and pending admissions/discharges, and adjusts assignments accordingly
  • Consults with the nursing staff concerning the implementation of patient's plans of care
  • Reviews doctor's orders and receives all new orders to coordinate appropriate changes or services
  • Attends and/or leads meetings with Nursing staff, Nursing Administration, and/or physicians
  • Identifies and implements staff development needs
  • Identifies and resolves obstacles associated with unit processes and procedures
  • Coordinates room changes with families and Admitting department
  • Completes patient review with MDS nurse, establishing and maintaining short and long term goals
  • Discusses patient needs with other departments, such as Physical Therapy or Recreation, and coordinates multiple services needed for patient care; coordinates contract therapies such as Occupational Therapy, Physical Therapy and Speech therapy
  • Provides professional nursing care by determining the kind and amount of nursing care required
  • Oversees the admissions and discharge processes to ensure a smooth transition for all patients
  • Makes periodic rounds to check quality of nursing care and to observe acutely ill patients
  • Contacts families of residents regarding issues that may arise such as medical or behavioral problems
  • Supports attending physicians with patient rounds. Makes, records, and discusses resident's progress with doctors and informs physicians of any important medical changes and conditions
  • Ensures orderliness and cleanliness of unit and proper care and maintenance of equipment
  • Functions as nursing representative on the unit during any licensing or accreditation survey
  • Leads weekly care conferences with subacute residents, family, and interdisciplinary team
  • Maintains appropriate resident documentation and files
  • Keeps current with related professional developments, regulations, and/or operational procedures impacting unit operations
  • Participates and leads in facility-wide quality improvement initiatives and projects including but not limited to Quality Assurance and Performance Improvement, Skilled Nursing Facility, Value Based Purchasing, and Quality Reporting projects

REQUIREMENTS INCLUDE

EDUCATION and/or EXPERIENCE
Completion of a Bachelor's degree in Nursing and five years Medical/Surgical or hospital nursing experience that included three years in a supervisory role; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above.

SUPERVISORY RESPONSIBILITIES
Exercises supervision of personnel in related area of responsibility.

CERTIFICATES, LICENSES, REGISTRATIONS
This position requires possession of a current license in Illinois as a Professional Registered Nurse (RN) Basic Life Support (BLS) by the American Heart Association.
