Client Care Coordinator

The goal of this role is to deliver the highest excellence of service to clients and advocate on their behalf interdepartmentally. This role assists incoming clients in the orientation process, provides additional clinical support throughout treatment, runs clinical groups, and completes intake and exit outcomes assessments, assists with other assessments such as BPS, intake assessments, LOCUS and CSSRS safety assessments. Additionally, it is the goal of this role to support the clinical team in the delivery of overall clinical services and ensuring the client experience is streamline and exceptional.
Core
Responsibilities:
Assist clients in getting acclimated to the treatment setting by providing a thorough orientation upon admission and conducting follow up check ins to ensure excellent client experience
Advocate on clients’ behalf by seeking out their
feedback/suggestions,
reporting on this to the Clinical Director and/or the other Department Leadership, as well as ensure the needs of clients are being met as it relates to their treatment, their environment, and their comfort.
Provide support to clients who are struggling with remaining present in treatment (i.e. AMA risks)
Provide feedback on client progress and/or behavior to respective therapists- collaborative effectively with the Clinical Team to provide support to the Team and the client
Attend daily morning clinical meetings
Interact with clients during breaks and throughout the clinical day, as needed, to continue to report back on the emotional temperature of the community
Complete intake and exit outcomes assessments with each client to provide compliance department with the clinical outcomes data
Assist the Clinical Team in running clinical groups, conducting BPS assessments, LOCUS and CSSRS safety assessments, as needed
Administer and gather weekly client satisfaction surveys weekly as well as provide Leadership with this feedback via email
Conduct exit surveys for each client discharging and report any concerns to Leadership from these surveys
Model appropriate boundaries with clients
Collaborate with Operations Manager and Supervisor on weekly recreational client activities
Provide support to Neuro team, as needed
Conduct case management services to assist case managers, as needed.
Follow all TTC policies and procedures
Updates job knowledge by participating in educational opportunities and completing 20 annual Continuing Education Units per FAC 65D-30.
Follow all regulations regarding client privacy including Federal and State Regulations (HIPAA)
Professional Experience, Essential Skills, and Knowledge
Preferred: Bachelor of Psychology, Social Work or other related field
CAP, CAC or CRSS certification or minimum of 2 years of experience working in Substance Abuse and Mental Health Treatment
