Collaborate, educate, and implement relevant clinical and care policies, programs, and guidelines in accordance with evidence-based best practices and state and federal regulations. Serve as the clinical subject matter expert, provides guidance, develops, and mentors the Director of Nursing Services and other clinical leaders. Identify and coordinate training opportunities related to the communities clinical capabilities, clinical offerings, QAPI process, State/Federal Regulations and Clinical Best Practice Guidelines. Possess knowledge of the QAPI process, developing a Plan of Correction, Quality Measures, 5 Star Rating, and the RAI process in conjunction with the requirements for clinical documentation that supports the care provided and claims submitted. Partner with community Administrator, Director of Nursing Services, Director of Clinical Reimbursement, and VP of Operations to ensure excellent clinical outcomes and stewardship. Maintains expected frequency of community visits, adhering to visit standards.
