The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.

We are seeking a Patient Navigator to join our dedicated team at Santa Monica Fertility, located in Santa Monica, CA. This is a full-time position, working fully onsite, Monday - Friday, 7:30 am - 5:00 pm, with required weekend and holiday rotations.

RESPONSIBILITIES:

  • Responsible for the coordination and organization of personalized patient treatment plans.
  • Consults with new patients to review and educate treatment process, timeline, and required testing.
  • Reviews all pertinent medical information and previous records, including overseeing requests for additional medical records if applicable.
  • Facilitates the development of the treatment plan, timeline, necessary teachings and injection training, calendar, medication orders, and clinic appointments.
  • Coordinates outside monitoring and facilitates Portland visits for domestic and international patients.
  • Oversees the treatment coordination between departments for individual patient care plans working closely with financial, embryology, and genetics.
  • Responsible for consultation, execution, and direction of patient care throughout treatment cycles under physician advisory.
  • Consult with new and returning patients to explain the treatment process, including a potential timeline for treatment and all required testing.
  • Provide individual consultations regarding specifics of each cycle, timeline, and key decisions to be made regarding optional testing, including recessive genetic carrier screening and preimplantation genetic testing of embryos.
  • Communicates with patients in person, via phone or Skype, and through various portal communications to arrange pre-cycle testing and treatment plans, as well as to share lab results and medication instructions.
  • Coordinates and facilitates pre-cycle testing and treatment cycle monitoring for domestic and international patients to be completed in-house or with outside monitoring facilities.
  • Review medical records and test results, consulting with physicians as needed.
  • Same day communication with patients informing and explaining lab and ultrasound results and medication instructions as advised by physician.
  • Develops, plans, and communicates treatment calendars and timelines.
  • Works with patient and insurance requirements in order to plan and order medications and fulfill treatment requirements.
  • Provides support for patients prior to, during, and following the cycle.
  • Plans and prepares future treatment cycles.
  • Communicates and works closely with physicians and medical teams regarding patient problems or concerns to provide optimal patient care.
  • Completes training and individual injection training as needed, educating patients on correct mixing and administration of injection.
  • Confirms and collects all necessary consent forms for each cycle.
  • Communicates treatment and cycle plans to other departments, including clinical medical teams, patient services, medical records, financial/billing, genetics, and embryology.
  • Maintain patient confidentiality and a professional patient/provider relationship.
  • Organize forms, methods, and protocols to ensure patient care is efficient, pertinent, and cost-effective.
  • Responsible for complete chart documentation, including phone calls, medications, lab results, cycle plans, and patient instructions.
  • Works rotating weekend and holiday shifts, meeting with patients in the clinic as necessary, and covering patient care and treatment coordination.
  • Competency with EMR (nAble).
POSITION REQUIREMENTS
  • Certified Medical Assistant (CMA) Certification required.
  • BLS Certified required.
  • 5+ years of demonstrated service excellence experience; prior progressively responsible experience coordinating patient care.
  • 1+ years of IVF Coordinating experience is required.
  • Women's Health Experience preferred.
  • Experience in or knowledge of scheduling, clinical terminology, insurance, and clinic protocols.
  • Commitment to patient-centered care and privacy.
  • Strong patient services and communication skills.
  • Basic Computer knowledge.


COMPENSATION & BENEFITS

$30.00 - $40.00 hourly

The compensation range for this position is between $30.00 and $40.00 per hour.Final compensation offers will be determined based on factors such as years of experience, skills, and qualifications. In addition to competitive compensation, we offer a comprehensive benefits package, including healthcare, dental, life, and vision insurance.
