Cardiovascular Disease Prevention Program Lead | HEALTH PROGRAM SPECIALIST II

Job Description and Duties
This position supports the California Department of Public Health’s (CDPH) mission and strategic plan by supporting chronic disease evidence-based intervention programs that promote healthy behaviors, healthy communities, and improve the prevention, diagnosis, and management of chronic disease.
Under the direction of the Health Program Manager II (HPM II), Chief, Program and Policy Section, within the Chronic Disease Control Branch (CDCB), the Health Program Specialist II (HPS II) serves as the lead of the Cardiovascular Disease Prevention Program (CDPP), funded by the California Preventive Health and Health Services Block Grant (PHHSBG). The HPS II plans, develops, implements, and monitors strategy implementation, evaluation, and deliverable completion to prevent and manage cardiovascular disease (CVD), with an emphasis on hypertension, a leading cause of death in California. The HPS II also serves as the CDCB communications coordinator and Program and Policy Section editor. Approximately 10% travel is required to attend in- and out-of-state conferences, site visits, and trainings.
