Responsibilities

The Case Manager Registered Nurse will work within an interdisciplinary team and collaborate with physicians, family members, nurses, and other hospital staff throughout the patient's continuum of care. The Case Manager will provide compassionate quality care while assisting patients and families from pre-admission through discharge planning

Qualifications

Job Requirements:
  • Must have a minimum of 3 or more years of acute care nursing experience
  • Must have a minimum of 2 or more years of case management experience preferably in an acute care facility
  • New Jersey State license that is active and in good standing
  • Must have a current BLS certification,
Preferred Job Requirements:
  • Bachelor's degree in Nursing required, Master's degree preferred
  • Certification in case management highly preferred
