About the Role

Oaks Integrated Care is expanding its Addictions team, and we're seeking a skilled Addictions Therapist to join our team. As a key member of our team, you will play a vital role in supporting our Addictions team in multiple counties. Our programs serve adults, children, and families, and your contributions will be essential to our success in providing treatment to our community.

Responsibilities
  • Provide individual, family, and group counseling to clients at an Intensive Outpatient Program (IOP) and Outpatient (OP) level of care, utilizing evidence-based treatment for Substance Use Disorder (SUD) populations.
  • Conduct comprehensive assessments through the New Jersey intake system, identifying individualized strengths, barriers, and needs of the person served.
  • Promote consumer-driven services and a recovery-oriented environment, assisting individuals and their families in making decisions, taking action, and treatment planning.
  • Provide education and information to individuals and families, as well as referrals and linkage to comprehensive case management programs addressing life domains such as social support, treatment needs, finances, transportation, legal services, and healthcare.
  • Work collaboratively with a multi-disciplinary team to coordinate services, including case management programs and prescribers.
  • Develop and implement individual recovery plans that are person-centered, including strategies for recovery, linkage to Medication-Assisted Treatment (MAT) services if desired, and identification of priorities, desired outcomes, and strategies and resources to achieve outcomes based on the assessment.
  • Provide training to clients on recognizing relapse triggers and strategies to avoid relapse.
  • Use engagement skills to motivate and encourage individuals to access services that promote a healthy lifestyle.
  • Advocate on behalf of individuals, providing a holistic experience.
  • Document all consumer sessions and outreach thoroughly and timely in the electronic care record, meeting applicable procedures, regulatory, and professional standards for timeliness, completeness, and qualitative aspects of care.
  • Participate in multi-disciplinary team meetings and individual supervisions per agency standards.

Qualifications
  • Master's Degree in Counseling, Social Work, or a related field (preferred)
  • Licensed by the State of New Jersey as a Licensed Associate Counselor (LAC), Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW); dual license with Licensed Clinical Alcohol and Drug Counselor (LCADC) a plus, pursuant to N.J.S.A.
  • One year of experience working with the behavioral health population (preferred)
  • Valid Driver's License
