Responsibilities

As a Case Manager, you will be responsible for the quality and resource management of all patients admitted to the facility. Your role will involve promoting cost containment, demonstrating leadership, and integrating healthcare providers to achieve seamless delivery of care. You will also perform a holistic and comprehensive admission and concurrent review of the medical record for medical necessity, intensity of service, and severity of illness.

Qualifications

To be successful in this role, you will need:

  • Minimum 5 years of work experience post-graduation of an accredited school of nursing and a current state Registered Nurse license.
  • Grandfathered prior to April 1, 2015, with a minimum of 5 years post-graduate of an accredited school of Social Work for Licensed Clinical Social Worker. RN Case Manager preferred.
  • Five years of acute care nursing experience preferred, with at least one year of experience in case management, discharge planning, or nursing management.
  • Current BCLS (AHA) certificate, preferred.
  • Knowledge of Milliman Criteria and InterQual Criteria preferred.
  • Experience and knowledge in basic to intermediate computer skills.

A reasonable compensation estimate for this role is $30.50 to $46.40, with the exact starting compensation determined at the time of hire based on a range of factors, including skillset, years of applicable experience, education, credentials, and licensure.
