Opportunity
We are seeking adedicated individual to fill the role of Community Outreach & Client CareCoordinator for our growing St. Charles office serving Kane and part of DuPageCounty. This position requires a proactive approach to developing relationshipswith referral sources and ensuring exceptional client care within our home careservices.
Responsibilities
- Develop and nurture relationships with referral sources such as hospital case managers, assisted living facilities, physicians' offices, and more.
- Execute a strategic sales plan to meet business development goals, including cold calls and face-to-face interactions.
- Achieve personal monthly and quarterly sales targets through proactive outreach efforts.
- Respond promptly to prospective client inquiries, whether in-person, by phone, or online, ensuring they receive necessary information.
- Organize and participate in local community events to enhance visibility and generate new business opportunities.
- Collaborate closely with office staff to ensure a seamless client experience and successful sales execution.
- Oversee client care plans to ensure completeness, accuracy, and alignment with client needs.
- Conduct in-home assessments for potential clients and develop personalized care plans.
- Supervise and participate in caregiver scheduling to ensure proper client/caregiver matches and adherence to policies.
- Conduct in-home quality service visits to maintain exceptional care standards.
Ensure compliance with all regulatory requirements, company policies, and procedures. - Manage client, caregiver, and outside care collaborator relations effectively.
- Share on-call responsibilities as needed to support business operations.
- Bachelor's degree preferred, with a minimum of 3 years of experience in a comparable marketing or management environment.
- Previous sales experience with a successful track record of identifying and building local relationships to drive business.
- Strong organizational skills with the ability to multitask and thrive in a fast-paced environment.
- Excellent communication skills, both written and verbal, with the ability to communicate clearly and concisely.
- Proficiency with Microsoft Office suite and willingness to learn new applications.
- Valid driver's license, proof of insurance, and ability to travel 80% of the time in the field.
- Ability to work some evenings and weekends as required.
- Self-motivated individual capable of working effectively with minimal direct supervision.
- Experience in home care or healthcare management is highly desirable.
- Must pass state and national background checks and be able to lift 30-40 pounds and use necessary job tools.
