Opportunity

We are seeking adedicated individual to fill the role of Community Outreach & Client CareCoordinator for our growing St. Charles office serving Kane and part of DuPageCounty. This position requires a proactive approach to developing relationshipswith referral sources and ensuring exceptional client care within our home careservices.

Responsibilities

  1. Develop and nurture relationships with referral sources such as hospital case managers, assisted living facilities, physicians' offices, and more.
  2. Execute a strategic sales plan to meet business development goals, including cold calls and face-to-face interactions.
  3. Achieve personal monthly and quarterly sales targets through proactive outreach efforts.
  4. Respond promptly to prospective client inquiries, whether in-person, by phone, or online, ensuring they receive necessary information.
  5. Organize and participate in local community events to enhance visibility and generate new business opportunities.
  6. Collaborate closely with office staff to ensure a seamless client experience and successful sales execution.
  7. Oversee client care plans to ensure completeness, accuracy, and alignment with client needs.
  8. Conduct in-home assessments for potential clients and develop personalized care plans.
  9. Supervise and participate in caregiver scheduling to ensure proper client/caregiver matches and adherence to policies.
  10. Conduct in-home quality service visits to maintain exceptional care standards.
    Ensure compliance with all regulatory requirements, company policies, and procedures.
  11. Manage client, caregiver, and outside care collaborator relations effectively.
  12. Share on-call responsibilities as needed to support business operations.
Qualifications
  1. Bachelor's degree preferred, with a minimum of 3 years of experience in a comparable marketing or management environment.
  2. Previous sales experience with a successful track record of identifying and building local relationships to drive business.
  3. Strong organizational skills with the ability to multitask and thrive in a fast-paced environment.
  4. Excellent communication skills, both written and verbal, with the ability to communicate clearly and concisely.
  5. Proficiency with Microsoft Office suite and willingness to learn new applications.
  6. Valid driver's license, proof of insurance, and ability to travel 80% of the time in the field.
  7. Ability to work some evenings and weekends as required.
  8. Self-motivated individual capable of working effectively with minimal direct supervision.
  9. Experience in home care or healthcare management is highly desirable.
  10. Must pass state and national background checks and be able to lift 30-40 pounds and use necessary job tools.
