Job Summary:The main goal of the Care Navigator is to enhance patient care experience by educating and guiding patients transitioning to an Essen Primary Care
Physician and managing relationships with insurance companies. The Care Navigator will generate awareness of our company and services, anticipate,
identify, and resolve service issues, while facilitating and improving internal/external communications. The Care Navigator will also track and analyze
existing referral patterns and reports trends to management. The Care Navigator will provide timely resolutions to issues that impact referral and
eligibility operations and will be charged to communicate outcomes promptly. Interacts with Executive Manager and other staff to relay referral source
concerns, and recommend and/or assist in the resolution of problems or service issues.
Additionally,the Care Navigator will be the secondary point person at the Front Desk and is responsible for establishing a high level of customer service
by monitoring the flow of personnel / visitors entering the facility, answering calls and triaging them, accordingly, handling incoming electronic faxes
and sorting through mail and paperwork given to the department.
Patient Education and Support:
Assist in educating patients on the transition process to Essen Primary Care services.
Help schedule appointments for patients with healthcare providers.
Provide general information about Essen services as needed.
Relationship Management:
Establish and maintain professional relationships with external partners to support collaboration.
Facilitate communication and assist in resolving issues with partners.
Patient Information Review:
Review patient information to assist in determining service eligibility.
Identify opportunities for patient outreach and education.
Keep patient records updated as required.
Roster Management:
Assist in reviewing patient rosters and maintaining accurate records.
Prepare basic reports on patient data as needed.
Secondary Front Desk Operations:
Monitor visitors and personnel flow within the facility.
Handle incoming communications and direct them appropriately.
Maintain a tidy and compliant work environment.
Administrative Assistance:
Support other departments with administrative tasks as directed by management.
Education
- Highschool diploma or GED
Skills
- Track record of exceeding activity and sales goals preferred.
- Healthcare background a plus.
- Problem solving and customer service skills required.
- Knowledge of healthcare and direct marketing to community agencies and/or physicians
preferable. - Sales/marketing specific background or education with focus on consultative selling.
Salary: $20.00- $25.00 hourly
