Patient Care Navigator

The mission of the Ambulatory Care Department is to deliver comprehensive, compassionate, and quality patient care. The mission is consistent with the hospital's mission, which is to deliver a wide variety of community-oriented health services/programs. Some task include but not limited to:
Serve as the primary point of contact for patients seeking outpatient surgery services.
•Schedule follow-up appointment
•Schedule consults
•Confirm appointment
Coordinate and schedule pre-patient appointments, diagnostic tests, and surgical procedures with follow through on recommendations from various consultants as needed with surgeon guidance.
. Maintain accurate and up-to-date patient records and documentation.
Triage patients as needed.
QUALIFICATIONS:
- Education Required - HS Diploma or Equivalence
- License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program
- Experience/Skills Required
- Previous experience in a medical setting or 2 years relevant work experience preferred
- Excellent communication/ interpersonal skills
- Bilingual preferred
- Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
