Good Shepherd Services, a leading multi-service agency, is seeking a skilled Youth Transition Specialist to join our team. As a specialist, you will work closely with young people aged 17-21 in foster care, providing them with the support and guidance they need to successfully transition into adulthood. With a focus on building long-term relationships and promoting self-sufficiency, you will be responsible for mentoring and providing services to address the unique needs of each individual.

About the Role

This is a full-time position that requires a strong understanding of the Child Welfare System and the ability to work effectively with a diverse group of young people. As a Youth Transition Specialist, you will be responsible for:

  • Managing a caseload of 8-10 young people at any given time
  • Providing individualized skill development and support to transition successfully into adulthood
  • Developing age-appropriate service plans with the young person and assisting them in achieving their goals
  • Working closely and collaboratively with interdisciplinary teams assigned to the young person
  • Escorting youth to various appointments as needed
  • Participating in agency training and receiving weekly individual development and clinical supervision
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a relevant field, such as social work or psychology
  • At least one year of experience working with at-risk youth or in a related field
  • Excellent written and communication skills
  • Ability to be flexible to the demands of young people participating in the LifeSet program
  • Ability to work autonomously as well as part of a team

Salary and Location

The salary for this position is between $50,000 and $55,000 per year, based on relevant post-graduate experience and location in the Bronx, NY. The position is onsite, with the possibility of hybrid work arrangements at the discretion of program leadership.
