What You Will Do
- Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care
- Initiate quality-driven care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently
- Practice independently in the community, traveling to patients' homes or care facilities, using effective time management and agility to manage weekly caseload and other duties
- Think critically to proactively organize, plan, and obtain supplies and equipment needed for patient care
- Complete individualized comprehensive assessments and related documentation, including but not limited to social determinants of health, OASIS assessments, wound care, and reporting vital sign changes
- Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community
- Provide empowering education and support to patients and their families to help them understand and navigate end of life treatment
Qualifications
- Current license to practice as a Registered Nurse in New York State
- Minimum of one year nursing experience in a medical, surgical, or hospice environment
- Valid driver's license or NYS Non-Driver photo ID card may be required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a team
- Must be able to walk up and down steps to access patient residences and transport required supplies weighing approximately 25 – 30 lbs.
