As a Field-based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed, as well as to provide home and social assessments and member education. CHW's work in a team-based structure and spend approximately 75% of their time in the community engaging directly with members. Interest in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.

If you reside within a commutable distance of Suffolk County, MA market, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Create a positive experience and relationship with the member
  • Help member set goals and develop a care plan to achieve those goals
  • Proactively engage the member to manage their own health and healthcare
  • Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
  • As needed, help the member engage with mental health and substance use treatment
  • Support the member to ensure access to member care needs, including SDOH needs
  • Provide member education on community resources and benefits
  • Conduct post discharge activities from hospital facility and support connection to social services
  • Support member to engage in work or volunteer activities, if desired, and develop solider social supports through deeper connections with friends, family, and their community
  • Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
  • Develop and implement individualized, person-centered care plans inclusive of goals, opportunities and interventions aligned with a person's readiness to change to support the best health and quality of life outcomes by meeting the member where they are

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Able to comply with UHG Recruiting Policy and have a designated workspace with access to high-speed internet
  • Reside within the local community for 2+ years, with knowledge of culture and values of community and familiarity with the resources available in the community
  • Reside within a commutable distance of one of the following Counties: Bristol, Plymouth, Suffolk
  • Reliable transportation and the ability to travel within assigned territory to meet with members and providers, locally, approximately 75% of the time (mileage is reimbursed)

Preferred Qualifications:

  • Bachelor's Degree (or higher) in a health-related field
  • CHW Accreditation
  • Field based experience
  • Care management experience
  • Experience working in Managed Care
  • Knowledge and/or experience with behavioral health or substance use disorders
  • Knowledge of Medicaid and/or Medicare population

Physical Requirements

  • Ability to transition from office to field locations multiple times per day
  • Ability to navigate multiple locations/terrains to visit employees, members and/or providers
  • Ability to transport equipment to and from field locations needed for visits (ex. laptop, etc.)
  • Ability to remain stationary for long periods of time to complete computer or tablet work duties
