At **HouseCalls Home Care**, our mission is to provide superior, compassionate home care workers and healthcare services for elderly and differently abled individuals in the comfort and dignity of their homes. We work closely with hospitals, physicians, and private caregivers for hire to develop and coordinate individual care plans, all while upholding our clients' independence at home.

We are currently seeking a dedicated and compassionate **Spanish-speaking Service Coordinator** to join our team. As a Service Coordinator, you will play a vital role in ensuring the smooth delivery of home care services to our clients. This position requires excellent organizational and communication skills, as well as the ability to coordinate care plans effectively.

**Responsibilities**:

- Coordinate and schedule home care services for clients based on their individual needs and care plans.
- Communicate with clients, caregivers, and healthcare professionals to ensure proper coordination and delivery of care.
- Maintain accurate and up-to-date client records and care plans.
- Monitor and evaluate the quality of care provided to clients and make necessary adjustments as needed.
- Serve as a liaison between clients, caregivers, and other healthcare professionals to ensure continuity of care.
- Provide support and guidance to caregivers as needed to ensure the highest level of service delivery.

**Requirements**:

- Previous experience in home care coordination or a similar role preferred.
- Strong organizational and communication skills.
- Ability to multitask and prioritize tasks effectively in a fast-paced environment.
- Knowledge of home care regulations and standards.
- Proficiency in relevant software and tools for scheduling and documentation.
- Bachelor's degree or equivalent experience in healthcare or related field preferred.
- Spanish-speaking required.

**Compensation**:

- Pay rate: $26 - $27 per hour depending on experience
