We are urgently hiring for a Mental Health Recovery Case Manager in **Austin, TX.**

Our Recovery Manager responsibilities include:

- Coordinates/leads development of the Individual Recovery Plan (IRP) using a PCRP which supports the individual;
- Actively coordinates with other individuals and/or entities essential to physical and/or behavioral services for the individual (including the individual’s MCO) to ensure that other services are integrated and support the individual’s recovery goals, health, and welfare;
- Initiates recovery plan team discussions or meetings when services are not achieving desired outcomes. Outcomes include housing status, employment status, involvement in the criminal justice system, response to treatment and other services, and satisfaction with services
- Advocates for continuity of services, system flexibility and integration, proper utilization of facilities and resources, accessibility, and beneficiary rights;
- Assists individuals with fair hearing requests when needed and upon request;
- Assists individuals with retaining PROGRAM and Medicaid eligibility;

Pay: $24.00 - $28.00 per hour
