Job SummaryWe are seeking a highly skilled Case Manager to join our team. As a Case Manager, you will be responsible for implementing treatment plans, coordinating care, and providing support to clients with complex mental health needs.
Key Responsibilities- Develop and implement individualized treatment plans in collaboration with clients, therapists, and other healthcare professionals.
- Coordinate care with social services, health agencies, and government organizations to ensure seamless delivery of services.
- Provide advocacy and support to clients, helping them navigate the healthcare system and make informed decisions about their care.
- Assess and address client motivational and psychosocial issues, promoting wellness and autonomy.
- Document all information accurately and timely, producing statistical reports as required.
- Collaborate with Deblin team members to ensure effective working relations and high-quality care.
- Masters Level Degree in a field of Mental Health Studies or Behavioral Science.
- Minimum of 1 year experience in providing mental health services, preferably in an outpatient setting.
- CPR Certification and ability to work with clients with higher needs (Level 3 or 4).
- Excellent knowledge of case management principles, healthcare management, and reimbursement procedures.
- Excellent organizational and time management skills, with ability to work remotely with minimal supervision.
- Familiarity with computers and other technical resources.
- Problem-solving skills and ability to multi-task.
- Bilingual (Spanish) a plus.
- Flexible schedule and remote work opportunities when clinically appropriate.
