**New Alternatives for Children, Inc. (NAC) is an innovative and interdisciplinary child welfare and social services agency that provides essential support to families with at least one family member who has a disability and/or chronic medical condition. Children and their families enter NAC’s services through the city’s foster care system, preventive services, or by way of community referral. All our clients have access to a wide array of services including intensive, multi-faceted support for children’s educational needs.**
- **Title of Position** **Family Team Conference Facilitator**:

- **Position Type**: **Full Time, Monday - Friday, hybrid schedule. **This position is typically 9am-5pm, with some evenings to accommodate client schedules (flexibility required).
- **Pay Range**: $50,000 - $65,000
- **Location**:825 7th Avenue New York, NY 10019 / Bronx Satellite Location - 1775 Grand Concourse Bronx, NY 10453**:

- **Department**:Special Medical Prevention Services**

**Description of Responsibilities**:

- Facilitate Family Team Conferences and pre-conferences with case planning teams.
- Collaborate with Quality Improvement and Program Assistant to track and send reminders to Case Planners of upcoming FTCs.
- Document FTCs in CONNECTIONS.
- Collect statistical data on FTCs as needed.
- Track action steps agreed upon during conferences and send reminders to Case Planners.
- Participate in weekly supervisory sessions, department meetings, clinical trainings, and social justice forums.
- Additional duties as assigned, based on the needs of the department..

**Required Education and Experience**:

- BA/BS/BSW/MSW/MHC or equivalent from an accredited school; and
- Two years of casework or group work experience

**Required Skills**:

- Family or group facilitation skills
- Organizational skills
- Knowledge of child welfare preferred
- Proficiency in Spanish preferred (and includes a salary differential)
