St. John's Residence for Boys is seeking a highly skilled and compassionate Family Team Conference Facilitator to join our team. As a key member of our multi-systemic service team, you will play a vital role in identifying needs and assessing appropriate tasks to achieve stated needs for resident youth and their families.
Key Responsibilities- Lead groups to formulate tasks and appropriate goals to address and assist in achieving permanency for resident youth.
- Facilitate the formulation of tasks and goals for the next six months towards the permanency plan, helping to identify who, what, and when to accomplish next steps.
- Liaise with the ACS Family Team Conference Team in scheduling FTC meetings.
- Collaborate with the multi-systemic service team to provide comprehensive support to resident youth and their families.
- Master's degree in Social Work or related field.
- 2 years of experience in child welfare or juvenile justice/residential care, or a minimum of 1 year of professional experience in child welfare, case management.
- Valid NYS Driver's License and personal transportation.
We are a child welfare agency serving youth throughout the NYC boroughs. Our agency operates 24/7, 365 days a year, providing options to accommodate a variety of scheduling needs for our staff.
