Personal Support Specialist Care Coordinator
- Directs and administers day-to-day activities of program in accordance with established policies to provide high quality health care services.
- Ensures economical and efficient performance under guidance of agency's goals, objectives, and operating budget.
- The successful Care Coordinator will be a dynamic professional who understands how to maintain the highest levels of client care and customer service while adhering to HIPPA guidelines and will demonstrate the ability to build relationships with employees, referral sources, and community resources to grow this Mainecare Certified Home Health program.
- The successful Care Coordinator needs to have a keen sense of attention to detail, be highly organized and be able to multitask in a very fast paced environment.
- Excellent written and verbal communication skills

**Responsibilities**:

- Collect weekly documentation from your Personal Support Specialist staff
- Complete quarterly home visits with all clients
- Complete monthly supervision with your Personal Support Specialist staff
- Complete initial intake assessments with clients and staff
- Develop personalized plans of care based on authorized services
- Manage a dynamic team
- Monitor quality and appropriateness of services provided
- Assure compliance with company policies and procedures
- Coordinate with referral sources and clients assigned Care Coordinators
- Assure compliance with applicable laws, DHHS and Mainecare regulations including section 96

Job Requirements:
Degree not required but preferred

Must valid drivers license and vehicle

**Salary**: $16-18 /hour

Benefits Include: Paid sick time, paid vacation time, mileage reimbursement, health insurance

Pay: $22.00 - $23.00 per hour

Schedule:

- 8 hour shift
- Monday to Friday

Work setting:

- In-person
