JOB TITLE: Medical Coordinator

**RESPONSIBLE TO: Program Manager**

**MAJOR RESPONSIBILITY**:The Medical Coordinator is

responsible for performing a variety of tasks related to QA of all sites; medications, attending and making medical/dental appointments, coordination of care with state officials; court appoints; transporting clients to events or school. Part of the QA team

**GENERAL DESCRIPTION OF DUTIES**:
The Medical Coordinator is responsible for developing a system to help and provide medical services to ensure wellness of residents. He or She will be responsible to maintain the medical information for coordination of care with state officials.

**EXAMPLES OF WORK**:
1. Upkeep interpersonal skills

2. Communication and cooperation with peers and other state personnel

3) Accompany and transport residents to activities such as:
a. Medical or dental appointments, etc.

b. Psychiatric appts

4.) Under the direction of a prescribing psychiatrist, responsibilities to include:
a. Monitoring the self-administration of medication to residents per policy

b. Responsible for monitoring health of residents and relaying health information to appropriate staff

c. Maintaining health records, including seizure reports, medication records, changes in medication; wellness of clients; menstrual records, etc.

5). As the Medical Coordinator, monitor and report resident’s progress:
a. Maintain individual records for medical appointments and all wellness of clients. Maintain accurate information for each resident on a daily basis for all activities.

b. Attend ITP/ART meetings for residents as designated

c. Attend scheduled staff meetings as designate

6. Professional advancement through a program of self-study to maintain current knowledge of current principles and program development within the professional field.

7 Provides new client with information on Psychiatric Medications if applicable. Coordinate with Program Coordinator for intakes and all new client medications.

8. Compiles all psychiatric medication information binder for reference.

9. Ensure all new clients receive literature on their medications and signs off on the client literature information sheet (Do verbal review with clients on all medications they are on.)

10. Sets up medical appointment for TB and Physical for all new clients within 24 hours of intake.

11. Additional duties as requested by MC, PM, and RPS.

**KNOWLEDGE, SKILLS AND ABILITIES**:
1. The ability to work with youth whom exhibit severe behavioral problems and maintain a positive demeanor.

2. The knowledge of basic behavior modification principles

3. The skills and knowledge to:
4. Protect client rights

5. Meet the unique needs of the client populations served by Ascend

6. Keep records and documentation of all medical needs of each resident.

9. Demonstration ethical behavior, such as staff and resident boundaries and the inappropriateness of receiving gratuities from a resident

10. Identify types of medications commonly prescribed for mental disorders, personality disorders, substance abuse, and common side effects and reactions to medications. Teaching clients about their prescribed medications and the side effects.

**HOURS OF WORK**: Forty (40) hours per week; exempt position

**EVALUATION SCHEDULE**: annual.

**QUALIFICATIONS**:
Must be at least twenty-one (21) years of age and have a minimum of:
1. Bachelors Degree in the social sciences and/or High School Diploma or MA certification with (2) years experience as an admin personnel; medical assistant; caregiver with admin experience.

2. Additionally, he/she must posses the personal characteristics necessary to effectively maintain a good relationship with state officials, Ascend staff and persons with emotional challenges.
