The Care Logistics **Referral Coordinator** is a healthcare professional, responsible for helping direct patients to the proper specialty provider referred by an RDH medical provider. This individual works to complete the assigned responsibilities and provides an appropriate medical environment to assure high quality care by performing the following duties.

**TYPE**: Full Time

**RESPONSIBILITIES**
- Focuses on patient priorities with a kind and caring attitude to ensure quality care.
- Maintaining Inbound and Outbound referrals for Redirect Health for all specialties:

- Schedule for the specialty services providers that come to our facility.
- Regular communication and follow up with the specialty service providers
- Understanding which insurances require a referral or an authorization
- Making sure that our referral tasks are up to date within a 72-business hour timeframe
- Documenting and logging all outbound referrals in EMR
- Documenting the dictation received from our specialty service providers
- Assist as a back-up for referrals if another Outbound Referral Coordinator is out*
- Answers patient inquiries in an appropriate manner and complies with patient's needs at all times.
- Performs general office responsibilities, to include typing and answering telephone/client inquiries.
- Responsibilities and accountabilities with KPIs (Key Performance Indicators) that clearly indicate how you'll determine, measure and know the employee is performing as required.

**REQUIREMENTS**
- Proficiency in using Microsoft word, Excel, and an Electronic Medical Record system, Preferred
- Bilingual (Spanish/English), Highly Desired
- Must be able to successfully pass National Agency Check with Inquiries (NACI) background investigation, Required
- High school diploma or general education degree (GED); one to two years related experience and/or training; or equivalent combination of education and experience.
