Summary:
Assists with implementing standards of care directly related to Clinical Practice Team policies and processes and assisting the assigned population with daily workflow according to the program.

**Responsibilities**:
Responsible for training new team nurses and navigators based on job description on all aspects of clinical practice team workflows

Responsible for being the team lead; reliably and efficiently answer and assist the team with related questions and workflows

Manage huddle(s) with the clinical practice team to organize and re-stratify staffing to support patient needs

Responsible for patient outcomes capturing and reporting in alignment with organizational standards and goals

Participates in chart audits for documentation compliance to meet standards of care and process improvement and development

Maintains integrity of identification of rising risk and high risk patients based on assessment utilization and internal algorithms and ensuring patients are seamlessly transitioned to appropriate care team or population health program

Works closely and effectively with direct supervisor to maintain integrity of the team while direct supervisor is unavailable

Prospectively and retrospectively works to identify and close gaps in care

Develop strong relationships with clinical teams providers practice managers and medical assistants to collaborate with practices on clinical practice team practices and procedures

Actively collaborates with manager and practices to improve patient outcomes

Attends and participates in team meetings

Attends and participates in leadership management operational and quality meetings as needed

Performs other job-related duties as assigned

Other information:
**Education and Experience**

Graduate of an accredited school of nursing and active Rhode Island license in good standing

At least 2 years of experience that is directly related to the duties and responsibilities specified

**Knowledge Skills and Ability**

Ability to identify patient needs directly related to his/her well-being and patient condition during telephonic outreach and providing the care necessary to assess educate provide reinforcement and/or escalate to the clinician

Ability to demonstrate initiative possession of strong organizational and analytical skills and attention to detail and efficiency is important

Ability to function as part of a team with other team members within the company

Able to maintain confidentiality with all aspects of information (including patient data) in accordance with the Lifespan policy

Must handle the most sensitive and confidential matters with the utmost discretion

Working knowledge of centralized clinical programs including population health and disease management programs centralized administrative programs such as referral management and document management and quality programs

Travel

State-wide travel will be required

Must possess an insured personal vehicle be willing to travel and hold a valid driver
