Supervisor: Director of Student Support Services

FLSA Status: Exempt

Regular Hours: Monday through Thursday, 12:00 p.m. to 8:00 p.m., and Friday, 8:30 a.m. to 4:30 p.m.

**Purpose of position**

Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to connect students and families with options and services to meet their comprehensive health and educational needs. Through proactive communication and knowledge of available and accessible resources, case managers reduce barriers to promote school attendance and optimal learning.

Related activities to case management include comprehensive case management, promotion of health and wellness, community collaboration, and facilitating care services. The individual in this position is responsible for 1) identifying students at risk for school failure associated with personal, familial, and/or school-related barriers, 2) assessing health, education, and social service needs, 3) developing an integrated school-community service delivery plan and 4) advocating on behalf of students.

**Qualifications**

Bachelor’s degree in social work or a related field, such as Clinical Mental Health Counseling, and possess three plus years of experience working in education, public health, human services, and/or other direct social service areas.

**Knowledge, Skills, and Abilities**
- Fluency in English and Spanish
- Strong advocacy and leadership skills in working with individuals and groups (e.g. initiate individual and group discussions, listen, clarify and facilitate interactions, and share ideas)
- Strong professional writing skills
- Ability to counsel students, parents, staff, and laypersons individually and in groups
- Ability to demonstrate effective liaison relationships with parents, schools, and agencies
- Communicate concepts and information accurately orally and in writing
- Ability and willingness to work with individuals from diverse backgrounds

**Responsibilities**
- Maintain a high level of ethical conduct regarding confidentiality, dual relationships, and professional stature.
- Maintain a fluctuating caseload of students in a nontraditional school setting, providing support to minors, adult learners ages 16-30, and first-generation college students accessing higher education.
- Contribute as a stakeholder to a collaborative school-wide team that delivers interventions that promote student attendance and academic success.
- Conduct residency verification and truancy home visits in collaboration with the Student Attendance Review Team (SART) and enrollment activities.
- Present case conceptualizations using various therapeutic modalities, such as Solution-Focused Brief Therapy (SBFT), Motivational Interviewing, Cognitive Behavioral Therapy (CBT), and other Trauma-Informed interventions.
- Coordinate, organize, and maintain electronic case notes and files for students assigned to caseload.
- Collaborate with stakeholders, such as the Director of Student Support Services, Case Managers, and the Career and Life Skills department, to identify relevant and appropriate community resources and develop workshops/training for all students and staff.
- Maintain communication and collaborative planning with specific supportive roles (e.g., Senior Case Manager, McKinney-Vento Liaison, Special Education Director/ Coordinators, etc.).
- Participate in monthly individual supervision and monthly team meetings with the Director of Student Support Services.
- Attend meetings and professional development activities as required to understand the social-emotional needs of students
- Conducts other duties as assigned.

**Major Areas of Accountability**
- Adhere to laws related to the DC Attendance Accountability Amendment Act, client confidentiality, and mandated reporting.
- Adheres to, implements, and performs within standards set forth by the Family Educational Rights and Privacy Act (FERPA)
- Adhere to laws related to DC Mandated Reporting
