Overview:
**Requirements**:

- Prior experience in primary care or related healthcare fields
- Proficiency in medical records management and patient service coordination
- Strong knowledge of medical terminology and infectious disease care
- Ability to provide compassionate patient care in various medical settings
- Excellent communication skills and the ability to work effectively in a team environment

**Duties**:

- Coordinate patient services and ensure the delivery of high-quality medical care
- Manage medical records and documentation efficiently
- Provide direct patient care, including medical-surgical and infectious disease care
- Demonstrate a deep understanding of physiology, medical terminology, and critical care practices
- Collaborate with healthcare professionals to optimize patient outcomes
- Conduct comprehensive initial assessments and develop personalized care plans for assigned members.
- Establish and maintain strong rapport with members, managing a caseload of up to 25 individuals.
- Assist members in accessing community-based resources, scheduling medical appointments, and completing necessary referrals for both community and health services.
- Document all interactions and activities accurately using the designated health portal.
- Conduct outreach tasks and conduct face-to-face visits with members in a clinical setting.

**Job Type**: Part-time

Pay: $19.00 - $24.00 per hour

Expected hours: 25 per week
