**Main Purpose of Job**:
The main purpose of this position is to protect and promote the health and safety of Talbot County's children and families by providing education and care coordination to Medicaid and Medicaid eligible clients, and to other clients in the Healthy Families America program. This program assures linkage to services and education as needed. Additionally, the purpose is to perform Family Surveys (assessments) according to the protocols of Healthy Families America program, as well as optimize the home environment for the physical, mental and emotional well-being of the child/children, assist on strengthening parent-child relationships improving parent skills, and reduction of family stress of overburdened parents.
**MINIMUM QUALIFICATIONS**:
**Education**: Graduation from an accredited high school or possession of a high school equivalency certificate.
**Experience**: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
**Notes**:
**DESIRED OR PREFERRED QUALIFICATIONS**:
- Knowledge of and/or experience with community resources available for families with children.
- Experience with individuals from various cultural backgrounds and beliefs.
