We are looking for

Residential Supervisor who can train, supervise and evaluate all direct support staff. Participate as a member of the Interdisciplinary Team in the development and evaluation of each resident's individualized Service Plan. Provide 24-hour coverage of the Residence and plan schedules for staff that meet the needs of the Program and individuals.

Essential Functions
- Follow appropriate procedures relative to the reporting of incidents as per the 624 regulations.
- Schedule direct support staff to accompany residents on all appointments and activities.
- Supervise direct support staff’s implementation and maintenance of training programming.
- Supervise the purchase and maintaining of residents’ clothing, personal items, adaptive and habilitation equipment. Maintain and account for each resident’s personal allowance ledgers, bankbook, deposits, withdrawals and fiscal records.
- Provide ongoing monitoring and supervision through weekly supervision with assistant and senior staff to provide guidance, troubleshooting and ongoing training to contribute to the professional growth of Staff.
- Facilitate the planning, development and implementation of person-centered plans; develops individuals goals and outcomes with people supported; coordinating and participating in individual plan meetings; monitors, documents and reports progress toward meeting outcomes; ensures supports needed are in place to meet outcomes; coordinates communications with other service agencies to ensure individual outcomes are met.
- Ensure the timely receipt and filing of the Interdisciplinary Team summaries/ reports from various disciplines.
- Ensure timely completion of summaries for monthly, semi-annual and annual reviews. Submit semi-annual and annual reviews to Service Coordinator for inclusion in the individualized Service Plan.
- Complete addendum/changes to Residential Habilitation Plans as per team agreement.
- Coordinate participants Residential Habilitation Services and collaborate with all clinical disciplines, which may include: Nursing, Speech/ Language, Nutrition, Psychology, Recreation, Medical, Pharmacy
- Notify team members, parents, guardians, relatives and residences of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.
- Participate in the development of Individualized Service Plan (ISP).
- Develop, review, revise and monitor program goals (Valued Outcomes) as per consumer progress, needs and choices and in a timely manner.
- Liaison with Schools, Day Program and outside service providers to ensure consumer services are integrated. Attend meeting with outside providers and other habilitation provides to ensure integration of services and accuracy in consumer’s strengths and needs.
- Direct and coordinate the activities of all program, treatment and teaching of individuals in assigned caseload.
- Oversee the development of integrated and coordinated active treatment program for each individual on their caseload.
- Oversee/develop the Quality Assurance plans to monitor the implementation of each program to ensure Active Treatment.
- Serve on various committees: human rights, incident report, etc

Pay: $62,400.00 per year
