Pay Range: $23.00 to $27.00 per hour

As the Scheduling Coordinator, you are directly responsible for scheduling the home visits for field staff and assisting with the coordination of patient care. This involves ongoing communication with field staff, patients and clinical manager regarding scheduling of patient services. Additional responsibilities include: maintaining contact with hospitalized patients, coordinating referrals, and ordering and maintaining supplies.

**Requirements**:

- High School diploma or GED equivalent.
