Purpose:

- The primary purpose of the Admission Coordinator position is to coordinate all admitting department operations while ensuring compliance with all applicable standards. This position works toward increasing the nursing home census and strives to promote/foster a good reputation on behalf of the facility throughout the community.

Job Duties:

- The Admissions Coordinator will be directly responsible to ensure all documentation in relation to any admission is completed.
- You will be responsible for ensuring a smooth transition for new residents and their families, as well as maintaining positive relationships with referring individuals and organizations.
- Coordinate and oversee the admissions process, including conducting screenings and assessments of potential residents.
- Develop and maintain relationships with hospitals, physicians, and other healthcare professionals to facilitate referrals.
- Provide guidance and support to prospective residents and their families throughout the admissions process.
- Collaborate with the nursing and administrative teams to ensure the appropriate placement and care for new residents
- Maintain accurate and up-to-date records of admissions, discharges, and other relevant information.
- Implement and enforce policies and procedures related to admissions, including compliance with regulatory requirements.
- Conduct tours and presentations for potential residents, families, and healthcare professionals.
- Participate in marketing and outreach activities to promote the facility and attract new residents.
- Assure seamless and customer-friendly care transitions.
- Delivers and secures the Admission Agreements, financial paperwork and contracts with clients.
- Greets new residents and family on admission.
- Enter new admission data into Point Click Care
- Assist with assessments/evaluations as needed.
- Maintain accurate admissions files and referral logs in an organized and timely manner.
- Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident.
- Work closely with Case Management to coordinate patient discharges.
- Review facility policies and procedures as part of the facility’s interdisciplinary team to assure compliance with state and federal regulations.
- Support the Social Service Director and in their absence, assume the authority, responsibility, and accountability of directing the Social Service Department.
- Issue and ensure proper notice is given and procedure is followed including time frames, for Notices of Medicare Non Coverage, Skilled Nursing Facility Advanced Beneficiary Notice and Transfer and Discharge Notices.

Position:
Full Time 80 hours bi-weekly

Experience/Minimum Job Requirements:

- Bachelor's degree in a related field (e.g., healthcare administration, social work)
- Minimum of 3 years of experience in admissions or a related role
- Knowledge of senior care industry regulations and best practices
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work collaboratively in a team environment.

- Proficient in Microsoft 365
