**Responsibilities**:
- Your responsibilities include making clinical observations about the medicines and communicating these with the pharmacist, doctors, or other caregivers.
- You may interview patients directly, look at their medical records, and review the information provided during admissions at the facility or a previous discharge.
- Perform medication reconciliation for patients admitted to the facility
- Collaborate with healthcare providers to verify medication orders
- Utilize medical records and pharmacy systems to accurately document medication histories
- Identify discrepancies in medication orders and work towards resolution
- Communicate effectively with patients and healthcare teams regarding medication information
**Requirements**:
- Prior experience in hospital or nursing home
- Knowledge of aseptic technique and medication administration
- Familiarity with inhalation medications and patient service
- RN License
- Understanding of critical care environments and medical terminology
**Job Types**: Full-time, Part-time
Pay: $40.00 - $50.00 per hour
Expected hours: 40 per week
