Job Summary

We are seeking a highly skilled and compassionate Medical Care Specialist to join our team at HarmonyCares. As a Medical Care Specialist, you will be responsible for providing high-quality, coordinated healthcare services to patients in their homes.

Key Responsibilities
  • Develop and implement patient care plans that address identified patient problems
  • Provide intermittent skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities
  • Communicate patient progress with other disciplines and direct, supervise, and instruct non-professional home health aide staff
  • Admit patients eligible for home care services within 24-48 hours
  • Perform OASIS time point assessments and submit recertification paperwork per Medicare criteria and agency policy
  • Supervise Home Health Aide and license and document per Medicare criteria and agency policy
  • Act as a preceptor in the orientation of new nursing staff
  • Participate in Process Improvement (PI) program and discharge planning process
Requirements
  • One year of experience as a home care professional nurse
  • Competency in performing home care comprehensive assessment
  • Proficiency in personal computer use, including clinical, word processing, spreadsheet, and presentation software
  • Able to lift 40 pounds from floor to shoulder
  • Able to drive a car 2-4 hours per day
