**Your Specific Responsibilities**:
This is a professional Health Education position in the field of Public Health working with the Family and Personal Health Field Teams in Collier County, planning, implementing and evaluating health promotion and Public Health Education programs for at risk pregnant women and infants. This position requires the individual to be self-motivated, and work independently away from direct supervision. Services are provided by letter, phone and in the home. This position also provides field service support to other Health Department Divisions as requested. Contributes to the organization’s culture of performance excellence by actively seeking to improve personal and organizational performance through measurement, analysis, knowledge management, and processes improvements and participation in performance management committees, councils, and/or workgroups. Continuously demonstrates the Department’s ICARE values. This position reports directly to the Community Health Nursing Consultant.

**Healthy Start Case Management**
- Provides care coordination services to eligible Healthy Start families according to the Healthy Start Implementation Standards and Guidelines.
- Client services are provided by letter, phone calls, home visits, medical facilities and other community outreach areas.
- Promotes basic health principles and preventive care related to disease control, maternal/child/fetal health and family planning.
- Provides education to families on child growth and development, nutrition principles and the benefits of immunizations.
- Provides appropriate referrals to client for community resources.
- Maintains an open case load as per the Healthy Start Contract’
- Performs expected number of home visits per month.

**Healthy Risk Assessments, Initial Intakes and Support**
- Provides information to potential clients about Health Start and does an Initial Contact to those identified as needing Healthy Start Services. Initial Intakes may be made at a variety of facilities such as TAPP, WIC, MD offices, Hospitals, etc.
- Provides translation support for the Healthy Start Program.

**At Risk Population**
- Client assignments may include issues related homelessness, poverty, migrant workers and illiteracy.
- Must be able to converse with a variety of cultures.
- Collaborates with the Department of Children and Families and other community partners in the follow up of referred at risk families.

**Other**
- Enter EARS and services into HMS according to agency protocols and timeframes.
- Attends and completes required in-services and trainings.
- Other duties as assigned.
- Assist with overseeing the documentation and training of paraprofessionals.

**Required Knowledge, Skills, and Abilities**:

- Knowledge of Maternal/child health
- Home visiting experience, preferably Healthy Start
- Ability to use Health Management System (HMS)
- Ability to deliver health education services
- Ability to plan, organize and coordinate work assignments independently
- Ability to communicate effectively, clearly and concisely through oral and written means

**Qualifications**:
**Required**:

- Healthy Start workers working as a professional, must meet one or more of the following educational requirements and have received all required training:

- Four-year college degree in one of the following areas:

- Social sciences
- A health-related field such as nursing, health education, health planning, or health care administration.
- Social Work
- Two years of college with three years of public health/maternal-child health experience.
- Bilingual in English and Spanish or Creole.
- This position is at risk of exposure to blood and airborne pathogens.
- This position is required to follow security guidelines established by the agency.
- Current driver’s license and reliable transportation and willingness to use personal vehicle for business related travel.
