Speech Therapist (SLP) - Home Health (PRN)

Job Summary

Empath Health is seeking a PRN Speech Therapist to provide treatment to patients in their homes. The ideal candidate will have experience in home health care and be able to work independently.

Responsibilities
  • Provide speech therapy in compliance with physician's prescription and patient's plan of care
  • Perform initial and ongoing assessments to identify client's level of functioning, swallowing and communication ability
  • Develop a plan of care in consultation with the physician and other care team members
  • Instruct patients and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders
  • Instruct patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language and cognition
Requirements
  • Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology
  • Licensure as a speech pathologist in the respective state
  • One (1) year home health care experience preferred
