**Summary**
The Resident Care Coordinator works with the Executive Director in directing, coordinating, and supervising services provide by the RMA and Care Staff. The Resident Care Coordinator will also handle medication management and coordinate with physicians on resident care.

**Duties**
- Supervises RMA and Care Staff
- Monitors Medication Plan
- Interfaces with physicians; home health, and hospice
- Interfaces with families and visitors
- Assists in monitoring the overall health and welfare of residents
- Coordinates schedules
- Works as RMA as needed
- Assist in the Management and Monitoring of Eldermark
- Participates in completing UAI/ISP as assigned.
- Must keep work areas neat and clean
- Has the ability to spend long periods of time on their feet and to ambulate quickly.
- Must be free of non-treated communicable disease.
- Must be accurate, dependable, neat, and thorough in assigned tasks.
- Ability and willingness to learn quickly.
- Must be willing to work with the elderly population.
- Must be capable of performing the duties and responsibilities of the job, with or without reasonable accommodation.
