Based in Denver and serving a growing number of patients across key regions, we are committed to innovation, quality care, and transforming the kidney care landscape.

Who You Are

  • Board Certified in Nephrology, Internal medicine, Geriatrics, or Family Medicine, with experience in chronic disease management. Specialty in Nephrology is advantageous but not required.
  • A proven leader capable of managing a multidisciplinary team and fostering a culture of excellence and compassion.
  • Skilled in building and maintaining productive relationships with partner groups and provider networks.
  • A strategic thinker with a track record of implementing performance improvement initiatives and leveraging data analytics for population health management.
  • A holder of medical licensure in Illinois (with eligibility for Indiana) and possibly an MBA, although not mandatory.

What You'll Do

  • Lead and inspire a multidisciplinary team in delivering exceptional care to CKD and ESRD patients, applying innovative population health and value-based care principles.
  • Drive regional performance to achieve and exceed goals, focusing on quality, optimization, and patient satisfaction.
  • Act as the primary clinical liaison, integrating our Client's Care Model across the region and ensuring seamless collaboration with key partners.
  • Engage in continuous learning and development activities, staying abreast of the latest in renal care, treatment guidelines, and healthcare policies.
  • Participate in quality improvement projects, data analysis, and strategic planning to enhance care delivery and outcomes.
  • Enjoy a competitive salary, performance-based bonus, equity options, and a comprehensive benefits package, including unlimited PTO, CME allowance, malpractice coverage, and a 401(k) match.

This role is more than a job; it's an opportunity to make a lasting impact on the lives of thousands while leading the charge in innovative kidney care. If you're a dynamic, compassionate leader passionate about making a difference, we want to hear from you. Join us in our mission to transform kidney care and improve patient lives across our region.
