Staff Mentor/Resident Care Coordinator

Under the direct supervision of the Director of Wellness, the Staff Mentor/Resident Care Coordinator is responsible for playing a critical role in ensuring new hires have all the necessary training they need. The Staff Mentor/RCC coaches, teaches, and provides input to the performance evaluation of all new direct care employees. The Staff Mentor/RCC also assists with maintaining the resident’s medical records and assuring the smooth functioning of the unit.
Essential Functions:
- Orientates new staff member to the department and facility at large, in their role.
- Explains role responsibilities and expectations in the facility.
- Teaches new staff day-to-day duties/responsibilities
- Cover all skills listed on the mentor checklist, and provides the proper evaluation as needed.
- Becomes a coach to new staff in their department and creates an environment for long-lasting mentorship.
- Provides feedback to new staff to better assist them with their training process and recommends additional training as needed.
- Completes medication administration competency check with all new hires prior to working on floor independently.
- Assist with new admissions, re-admissions and assist with the transfer of Residents to different rooms within the facility.
- Maintains a positive and professional demeanor toward residents, visitors, families and co-workers.
- Works closely with care staff, physicians and residents’ families in order to provide the best care for the resident. This includes, but is not limited to, reporting resident concerns to the MD/family/care manager, reporting incidents to applicable parties, documenting on incidents/changes of condition, requesting orders.
- Assist with assessments and development of ISPs for residents in coordination with the DOW. Monitor assessment schedules within PCC to ensure they are up to date. Assist with updating ISPs for residents as needed d/t changes in condition or increased needs. Assist with reviewing care plans with residents, families, and care managers.
- Maintains all residents’ charts and ensures they are complete and up to date. Assist DOW with developing chart for resident within point click care.
- Maintains all medical records according to state regulations as directed by the Director of Wellness. Completes incident reports as needed.
- Checks Medication Administration Record daily. Enter orders into MAR/PCC as needed. Follows up on medications not in facility to ensure they are ordered. (includes diabetic supplies)
- Assist with testing of residents/employees as needed, such as COVID testing.
- Audits charts regularly noting deficiencies and/or discrepancies.
- Provides supervision to caregivers. Works with DOW to provide training and corrective counseling as needed.
- Maintains confidentiality of resident information.
- Ensure staff have access to resident records including ISPs, face sheets, folders for appointments.
- Monitor call light system for extended call lights times and notify DOW of concerns to follow up on.
- Coordinate appointments and transportation for residents. Performs medication cart audits, at a minimum, weekly.
- Make sure appointments are in PCC and transportation for residents is confirmed.
- Provide tours to prospective residents or their families when the Executive Director and/ or Marketing Director is not available.
- Bring concerns regarding team members or residents to the attention of the Executive Director and Wellness Director.
- Monitor and record health and behavior of residents and communicate such information with other management and staff.
- Perform medication passing and record keeping duties, if assigned (must be certified).
- Maintain the confidentiality of resident and client records, including identity, behavior and medical conditions and be cautious of discussing resident issues in front of others.
- Supervise and monitor resident safety, following applicable resident care policy procedures and standards.
- Read and implement residents’ individual care plans.
- Familiarize self with the Policy and Procedure Manual; be prepared to implement its contents and/ or support the Executive Director and leadership team with implementation of its contents.
- Know the emergency and evacuation procedures established for the facility.
- Attends all required training, in-service and staff meetings.
- Follows company uniform and grooming standards.
- Maintains a safe working environment.
- Adheres to all policies and procedures of Eden Senior Care.
- Other duties, as assigned
- Job duties are subject to change based on needs of the building
Qualifications/Skills/Educational Requirements:
- High school diploma or general education degree (GED). At least one year of experience in a health care facility and knowledge of medical terminology.
