The MDS Specialist is a remote position serving Prestige Care Skilled Nursing Centers in Oregon & WA

Applicant must have one year of MDS experience/licensed and live in Oregon or WA

Status: Full Time/Non-Exempt

Wage Range RN: $48.00 - $60.00 per hour DOE

Wage Range LPN: $42.00 - $55.00 DOE

POSITION SUMMARY

The MDS Specialist facilitates the completion of resident assessments in partnership with the Administrator, clinical team, and the Utilization department and ensures appropriate clinical services are provided and optimal reimbursement is captured for each resident and in accordance with accepted standards of nurse practice and federal and state regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

* Participates in the ongoing care in collaboration with the physician, Director of Nursing Services (DNS), clinical team members, social services, physical therapy, the resident, and the resident's family by tailoring care to meet individual needs of the resident.
* Partners with the clinical leadership team to facilitate the assessment and care planning process through the completion of the MDS, as scheduled for each resident. Ensures the MDS is completed accurately, timely and in accordance with federal and state regulations and nursing practices.
* Partners with the Utilization department to ensure that the resident record accurately captures the clinical services provided and optimizes reimbursement.
* Reviews and completes resident records and related documentation in a timely manner, including facilitating the completion of nursing assessment for each resident within 14 days of admission.
* Develops nursing care plans for each resident that meets the needs of the resident and the physician recommendations and reports changes in condition of residents that requires nursing intervention notifying the physician, director of nursing services, and family.
* Understands the correlation between the MDS coding and the impact on quality measures and works as part of the clinical leadership team to improve quality measures.
* Assists with new admissions, transfers, discharges, and ongoing resident care by ensuring documentation is current, signed, and the recordkeeping meets federal and state regulations, and company policies and procedure.
* Acts as a subject matter expert on the MDS, PDPM, and case mix.
* Follows all clinical policies and procedures, maintains HIPAA compliance, and works within the scope of their license and practice.
* Promotes a positive physical and psychosocial environment for the resident that upholds residents' rights and protects residents from all forms of abuse. Reports all incidents of resident abuse or suspected abuse immediately and completes required incident reports and investigations. If suspicion of a crime is suspected, reports to survey agency and at least one local law enforcement entity within a designated time frame by e-mail, fax, or telephone.
* Participates in leadership team meetings such as stand up, Managing Acute Care Conditions (MACC), Triple Check, and Utilization Review (UR). May be asked to attend other meetings such as Nutrition at Risk (NAR), and Quality Assurance Performance Improvement (QAPI) as needed.
* Attends and participates in continuing education programs, training, in-services, and staff meetings and keeps CEUs up to date.
* Maintains up-to-date knowledge of all rules, regulations, and policies pertaining to resident care. Responsible to stay informed of nursing practices and standards and apply new knowledge to meet the changing needs of residents.
* Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

* Minimum of 5 years of specialized MDS experience required, 10 years preferred.
* Must have a current, unencumbered nursing license in the state of practice.
* RN licensed strongly preferred. LPN license may be considered with significant specialized MDS experience and proper RN oversight.
* RAC-CT required or the ability to get certified upon hire.
* Strong knowledge of MDS, case mix, and PDPM required.
* Strong attention to detail required.
* Must be a team player, well organized, and flexible.

EDUCATION

* Bachelor of Science in Nursing, or Associate Degree in Nursing.

PTO:

SNF/AL/PCI Salary: Accrue PTO at rate of 0.0615/hour during their first year/16 days.

SNF/PCI Hourly: Full-time and Part-Time team members (over 20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.
