Nurse Practitioner

Kaiser Permanente is seeking a highly skilled Nurse Practitioner to join our team in Vancouver, Washington. As a Primary Care Psychiatric Nurse Practitioner, you will play a critical role in providing high-quality health care to patients with mental health needs.
Key Responsibilities- Provide assessment and follow-up consultation for Primary Care MDs when mental health issues are present and beyond the scope of practice of primary care physicians.
- Support smooth transition or stabilization of members discharged from mental health care back into primary care for follow-up and ongoing management.
- Coordinate with the mental health department and represent the point of contact resource for primary care in coordination of care for members with co-occurring mental and physical illnesses.
- Perform health maintenance services, including patient education, for well members.
- Document in writing all care in patients' permanent medical record in accordance with accepted medical/legal standards.
- Minimum two (2) years of experience providing services within a mental health clinic or agency.
- PMHNP from an accredited program or equivalent requirements to meet State Licensure.
- Registered Nurse License (in the state where care is provided).
- Nurse Practitioner License (in the state where care is provided).
- Nurse Practitioner National Certificate from American Association for Nurse Practitioners.
- Drug Enforcement Administration Registration.
- National Provider Identifier required at hire.
