* Implements agencies program for “Full Circle Continuity of Care and Parallel Service Models”, policies and procedures and follow transitional homes living setting protocols.
* Must be able to follow the “Comprehensive Community Recovery and Re-integration (CR2)” program for Justice-Involved Services (JIS) referrals and Choice Recovery Path models
* Support all Krossroads programs and partner programs
* Build a collaborative relationship with upper management and development program tracking tools, measure program efficacy, and maintain an outreach resources list and active referral list
* Provide leadership to their assigned group(s)
* Collaborate on both micro and macro levels of agency quality assurance plan for Comprehensive Community Support Services (CCSS) caseworkers
* Support implementation of IOP services with all assigned clients and provide services for IOP
* Must be able to navigate electronic health record system - Advanced MD, have excellent digital literacy, organization, and planning skills
* Support HSD audit requirements
* MATRIX Training
* IOP experience

Direct Services:

* Collaborate with the administrative team to schedule clients for all services
* Provide direct services to clients, including but not limited to skill development, building on strengths, identifying natural supports, modeling positive social skills, increasing health and well-being, and any additional support as needed for IOP and client caseload
* Maintain a caseload for IOP groups and individual SUD counseling for clients that may include ASAM, DSM V and other evaluation tools
* Ensures client and staff safety
* Supports clients management
* Follows agency incident reporting protocol
* Coordinate wrap-around services in the community
* Refer clients to resources
* Coach clients to learn recovery skills thereby supporting a least restrictive care setting
* Provide examples of methods for interventions with evidence based modalitues
* Provide, education, and support to maintain state licensure requirements
* Lead IOP skills groups that may include trauma informed care, educational/skills groups for clients in areas such as basic skills, mental health, and addiction recovery.
* Utilizes ASAM to support client’s readiness for change goals
* Empowers each client to make his/her own life decisions
* Complete ASAM assessment, and updates as required
* Review and complete and updates IOP treatment plans as needed, or within no less than a 90-day timeframe
* Assists staff to formulate and achieve goals
* Participates in treatment team meetings
* Maintains healthy boundaries with staff and clients, and others that may be involved in their lives
* Is physically able to complete job duties
* Proficiency in digital literacy including Microsoft Word, e-mail, telemedicine, and EHR (electronic health record) and guide team in these areas as needed
* Can use the Seeking Safety, Trauma Informed Care, MATRIX, CRP curriculum as a tool to support client empowerment, safety, and insight to increase progress towards goals as applicable

Qualifications

Must be 18 years of age or older and;

* Must have reliable transportation.
* Must have degree in related studies and hold the LSAA license or the LADAC license
* IOP experience and behavioral health experience preferred
