*Duties: What you will do:*
* Continue to develop Unity Hospice’s palliative care program
* Manage a caseload of palliative patients
* Assist with all aspects of patient care including: diagnosis, treatment, medication management, assessments, and consultations
* Maintain comprehensive knowledge in field of hospice and palliative care
* Travel to patients wherever they reside: Home, nursing facility etc.

*Requirements: What you need to have:*
* Valid Advanced Practice Registered Nurse (APRN) license
* Valid Registered Nurse (RN) license
* NPI number, or ability to obtain one
* Controlled Substance License and DEA Number (preferred)
* Willingness to enter into collaborative agreement with Hospice Medical Director
* Driver’s license, insurance and vehicle
* 1-2 years of nursing experience (preferred)
* Utmost compassion and integrity
