Palliative Care Nurse Practitioner
We are in search of a dedicated and empathetic Nurse Practitioner to join our team, playing a vital role in delivering palliative care to our patients. In this position, you will collaborate closely with the Palliative Care Medical Director and other healthcare professionals to develop personalized treatment plans and provide exceptional clinical care to those in need. Your responsibilities will include conducting thorough assessments, formulating care plans, prescribing medications, and ensuring effective communication with the care team to achieve optimal patient outcomes.
Qualifications:
- Master's of Science in Nursing from an accredited institution
- A minimum of two years of experience as a Nurse Practitioner
- Preferred specialization in Family or Adult Medicine
- Current Nurse Practitioner certification/license
