Position Overview:

We are seeking a skilled Nurse Practitioner to deliver primary and palliative care to homebound patients in the Fairfax region. In this role, you will utilize your medical expertise under the guidance of a supervising physician, addressing the needs of a diverse patient population with chronic health conditions.

Key Responsibilities:
  • Deliver high-quality care to primary and palliative patients in the designated area.
  • Conduct comprehensive health assessments through physical examinations and detailed medical history reviews to formulate optimal care plans.
  • Communicate effectively with office personnel and clinical teams across various platforms.
  • Provide patient-centered care to a designated panel of patients.
  • Participate in quarterly medical staff meetings.
  • Develop clinical impressions, diagnoses, and treatment strategies.
  • Build trust and consult with patients, families, and caregivers during visits.
  • Order necessary diagnostic tests as per medical requirements.
  • Prescribe medications and other medically necessary interventions.
Qualifications:
  • Board Certified with a valid Nurse Practitioner or Physician Assistant certification in Virginia.
  • Demonstrated clinical skills with the ability to assess, diagnose, and treat effectively.
  • Positive attitude and strong interpersonal skills.
  • Ability to problem-solve independently and confidently.
  • 1-2 years of experience in primary care, particularly with geriatric patients; hospice experience is preferred but not mandatory.
