Whether someone is dealing with a crisis or long-term difficulties, our aim is to support their healing journey and help them live more fulfilling lives.We are seeking a dedicated Patient Care Advocate to join our team in Stamford, where you'll work collaboratively with clinical staff to ensure a comprehensive and compassionate approach to patient care.

Our Patient Care Advocates:
  • Conduct thorough assessments of patient concerns and expectations, ensuring alignment with Mindful Care processes to enhance patient satisfaction and compliance.
  • Maintain effective communication with patients, pharmacies, referral sources, hospitals, and therapists to meet patient needs and optimize retention.
  • Support patients in obtaining medical records, completing necessary forms, managing prescription refills, and facilitating GeneSight testing.
  • Secure prior authorizations from insurance companies for medications and services.
  • Connect patients to the appropriate departments for their inquiries, anticipating the need for emergency interventions when necessary.
  • Provide referral information to patients upon discharge from Mindful Care.
  • Perform administrative duties and other tasks as assigned by management.
  • Serve as a liaison between Mindful Care Patient Experience Representatives, Patient Operations, and psychiatric providers. 

Qualifications:
  • Required: 3+ years of experience in administrative functions or case management.
  • Preferred: Possession of BLS or CPR/First Aid certification.
  • Preferred: Experience with Transcranial Magnetic Stimulation (TMS) administration.
  • Required: Proficiency with Google Suite and Microsoft Office (Word, Excel, Outlook, Teams).
  • Required: Associate degree required; bachelor’s degree preferred (experience may substitute for a bachelor’s degree).
  • Required: Excellent verbal and written communication skills.
  • Required: Reliable transportation and home internet access.
Position & Schedule:
  • Type: Regular Full-time, Non-exempt.
  • Hours: Monday-Thursday, 8:00am-6:00pm.
  • Location Type: 100% in office.
  • Base Salary: $20.00 per hour.
