Position: Bridges Liaison (Young Adult Case Manager)
Location: Primarily based in Canton, OH, with occasional travel to North Olmsted, OH.
Type: Full-Time
About the Role:
As a Bridges Liaison, you will play a pivotal role in guiding young adults through critical life transitions. You'll manage a caseload of up to 15 participants, offering personalized support in areas such as enrollment, housing assistance, life skills development, and case management. Your goal will be to build strong, respectful relationships with participants, ensuring they have the tools and resources needed to succeed. This role also involves maintaining accurate documentation, attending court hearings, and collaborating with a diverse team to provide the highest quality care.
Key Responsibilities:
- Facilitate enrollment and manage continued eligibility for participants.
- Assist with housing needs and life skills development.
- Maintain up-to-date and accurate case documentation, including all visits, calls, texts, emails, etc.
- Ensure timely completion of assessments, case plans, and reviews.
- Attend court hearings with participants as needed.
- Maintain documentation in SACWIS and meet all training requirements.
Qualifications:
- A post-secondary program in social work, human services, behavioral health, or a related field, with at least two years of experience working with families and/or children in the child welfare system; or
- A bachelor's degree in social work, human services, behavioral health, or a related field.
- Demonstrated ability to work effectively with diverse populations.
- Valid driver's license, good driving record, and automotive liability insurance.
- Must have a safely maintained vehicle for travel.
- Ability to pass the background screening as defined in OAC code 5101:2-5-09.
- Sensitivity to cultural differences among children and families served by Pathway.
