Role: Young Adult Support Coordinator
Location: Primarily based in a designated area, with occasional travel required.
Employment Type: Full-Time

About the Position:

As a Young Adult Support Coordinator, you will be instrumental in assisting young adults during significant life transitions. You will manage a caseload of participants, providing tailored support in various areas, including enrollment, housing assistance, life skills enhancement, and comprehensive case management. Your objective will be to cultivate strong, respectful relationships with participants, ensuring they have access to the necessary tools and resources for success. This role also entails maintaining precise documentation, attending necessary hearings, and collaborating with a diverse team to deliver exceptional care.

Key Responsibilities:

  • Facilitate enrollment processes and oversee ongoing eligibility for participants.
  • Assist participants with housing needs and life skills development.
  • Maintain accurate and up-to-date case documentation, including all interactions.
  • Ensure timely completion of assessments, case plans, and reviews.
  • Attend necessary hearings with participants as required.
  • Document all information in the appropriate systems and meet all training requirements.

Qualifications:

  • A post-secondary program in social work, human services, behavioral health, or a related field, with a minimum of two years of experience working with families and/or children in the child welfare system; or
  • A bachelor's degree in social work, human services, behavioral health, or a related field.
  • Proven ability to work effectively with diverse populations.
  • Valid driver's license, good driving record, and automotive liability insurance.
  • Access to a safely maintained vehicle for travel.
  • Ability to pass the required background screening.
  • Awareness of cultural differences among the children and families served.
